Modern health practise is increasingly focused on treating the needs of the individual and their symptoms. This is somewhat at odds with the research that holds that sweeping positive changes to the social determinants of health are bought about by awareness and cultural behaviour. Interventions focused on an entire population, have a far greater impact on total population.
Collective action considers the three local government areas of Corangamite, Moyne, and Warrnambool’s Community Visions, Council, and Municipal Public Health and Wellbeing plans, along with the Healthy South West Integrated Health Promotion plan formed by current partners from across the region which is underpinned by the Victorian Public Health and Wellbeing Plan.
South West Primary Care Partnership is dedicated to this concept and supports many programs that have a focus on improving health, at scale, across the entire population of our catchment area.
