About Us
PCP’s have been in place for 9 years now and have undergone significant transition during this time. In earlier years the focus was on partnership development and planning. This progressed further and two new key initiatives of Service Coordination and Integrated Health Promotion were implemented and these areas were the key focus for a few years.
There have been a range of system and practice changes that have developed and this includes the introduction of common screening and referral tools, the establishment of processes and IT infrastructure for efficient sharing of information between service providers, supporting a Statewide Services Directory and the implementation of a planning framework for Integrated Health Promotion programs.
In more recent years there has been further expansion into the area of Integrated Chronic Disease Management with a focus on increased integration between general practice and primary health care providers and increasing self management of chronic conditions.
In 2009/2010 our focus continues on the above and other new areas of development include Gamblers Help health promotion strategies to reduce problem gambling, Elder Abuse Prevention Strategy project to increase education and awareness and active planning assistance to member agencies through the provision of population health data.
Our new Strategic Plan 2009 – 12 has been developed following an extensive planning and consultation process and the identification of 3 Strategic Priorities:

Obesity – to halt the rising trend of obesity

Social connectedness – strengthen the community to provide social connection, interaction and access to support and information

Health Equity – to reduce health inequality in vulnerable communities
These priorities will be our main focus over the next 3 years in line with the key deliverables of Service Coordination, Partnerships, Integrated Health Promotion and Integrated Chronic Disease Management.
